What Is a MedsCheck?
A MedsCheck is a one-on-one consultation between you and your pharmacist. It is designed to identify and resolve drug interactions, confirm each medication is still appropriate, review side effects you may be experiencing, clarify correct dosing and timing, reduce unnecessary medications, and improve your overall medication adherence.
Who Is Eligible?
- Ontario residents taking three or more medications for one or more chronic conditions
- Diabetic patients (eligible for annual Diabetes MedsCheck)
- Patients recently discharged from hospital needing a follow-up medication review
- Residents of long-term care homes or transitional care settings
What to Bring to Your MedsCheck Appointment
Bring all your medication bottles — prescription and over-the-counter — including vitamins, supplements, and natural health products. The more complete the picture, the more useful the review.
What Happens After?
You receive a Medication Management Plan — a written summary of your medications, any changes recommended, and follow-up actions. We share the plan with your physician if you consent.
