Wishing Well Pharmacy is a community pharmacy located inside the Wishing Well Health Centre in Stouffville, Ontario. We are committed to protecting the privacy and security of your personal health information in accordance with the Personal Health Information Protection Act, 2004 (PHIPA) and the federal Personal Information Protection and Electronic Documents Act (PIPEDA) through our pharmacy privacy policy.

What Information We Collect

We collect only the information necessary to provide pharmacy services, including:

• Your name, address, date of birth, and health card number
• Prescription details and medication history
• Medical conditions, allergies, and insurance information
• Payment information (when you purchase products or services)

How We Use Your Information

We use your personal health information to:

• Dispense prescriptions and provide compounding services
• Conduct medication reviews and compliance packaging (blister packs)
• Bill private insurance plans and government programs (OHIP, ODB, etc.)

Retention and Destruction

We retain prescription records and personal health information for the time required by Ontario College of Pharmacists regulations (generally 10 years from the last entry or 10 years after the patient’s 18th birthday). After that, records are securely destroyed.

Your Rights

Under PHIPA and PIPEDA, you have the right to:

• Access your personal health information (request a copy)
• Request corrections to inaccurate or incomplete information
• Withdraw your consent for certain uses (subject to legal or contractual restrictions)
• File a complaint with our Privacy Officer or the Information and Privacy Commissioner of Ontario
